SPS Healthy Youth Survey

Washington State Healthy Youth Survey 2021

Our schools are participating in the Washington State Healthy Youth Survey during fall 2021. The survey will be given to students in Grades 6, 8, 10 and 12.

The Healthy Youth Survey includes questions related to physical activity and diet, unintentional and intentional injury, substance use, sexual behaviors, abuse, risk and protective factors, and access to school-based services. Survey results are used by schools, communities and state agencies to plan programs to support our youth and reduce their risks.

Participation in the survey is voluntary and the students’ answers are anonymous.

Schools will be administering the survey online this year. No privacy issues were identified in the E-survey pilot conducted in 2019. This year, schools and districts are being instructed to disable any recording, keystroke tracking, and video capture software on school-issued devices used to take the survey.

Please read the Parent and Student Notification Letter for more information about the Healthy Youth Survey.

If your student attends one of our 11 comprehensive high schools, please also visit the Youth Risk Behavior Survey page to learn about another survey opportunity for your student.

About the Healthy Youth Survey

The Healthy Youth Survey (HYS) is a collaborative effort of the Office of the Superintendent of Public Instruction, the Department of Health, the Department of Social and Health Service’s Division of Behavioral Health and Recovery, the Liquor and Cannabis Board, and the Department of Commerce.

Seattle Public Schools participates in this survey every even year in October. The survey is administered to students in 6, 8, 10, and 12 grade. Student participation is completely voluntary and anonymous. Parents are notified about the survey and given the option to withdraw their student from participation.

The Healthy Youth Survey provides important survey results about the health of adolescents in Washington. Seattle Public Schools, along with County prevention coordinators, community mobilization coalitions, community public health and safety networks, use this information to:

  • Learn the prevalence of health-related behaviors among students
  • Evaluate or improve existing school programs, projects, and school health education programs
  • Support modifying or creating new policies that promote health
  • Track progress toward meeting school health and health promotion program goals
  • Understand the school climate
  • Contribute to the Continuous School Improvement Plan process
