Register for SPS Athletics with FinalForms

Seattle Public Schools (SPS) is excited to welcome families to another great year of school and athletics! If your student is not interested in participating in SPS athletics, please disregard this information.

To make athletic registration easier, the district is continuing to use FinalForms—an online form management system—for most middle, K-8, and high schools. (Note: Denny Middle School and K-8s Louisa Boren STEM, Broadview-Thomson, Orca, and Pathfinder are not using FinalForms.)

Why FinalForms?
FinalForms lets families complete and sign all athletic forms digitally and saves information year-to-year. It also pre-populates forms for each of your students wherever possible.

Families can review and update information anytime and will only need to sign forms once per year or after any updates to personal information.

Already registered?
Families and students who have already used FinalForms can log in with existing credentials.

New to FinalForms or SPS?
On Tuesday, Aug. 12, all SPS families of students in grades 6-12 who are new to the platform will receive an email from FinalForms inviting them to complete their registration. Families only need to complete registration if their student is interested in participating in athletics.

Students new to FinalForms will also receive an email from the platform inviting them to register. This will be sent to their SPS student email address.

Not sure if you already have a FinalForms account?

Not a problem. Try logging into the FinalForms website using the same email address you shared with SPS and your student’s school.
