Upon request from a parent or an adult student, home or hospital instruction may be provided to students who are unable to attend school for an estimated period of four (4) weeks or more because of disability or illness. A qualified medical practitioner must complete Section 1 of the Request for Home/Hospital Instruction form. This section includes the diagnosis, length of time the student is expected to be unable to attend school, and the practitioner’s contact information, printed name and signature. A request on letterhead that includes that information will be accepted in lieu of the form. The district shall not pay for any costs incurred in securing the medical verification.
