    COVID-19 Vaccination Opportunities

Seattle Public Schools wants to ensure that all students, staff, and families have access to the COVID-19 vaccine.

SPS is excited to partner with the City of Seattle, Public Health – Seattle & King County, and Seattle Fire Department to host COVID-19 vaccination clinics for students 12 and older, so students can be fully vaccinated and protected from the COVID-19 virus before in-person graduation and in advance of a full-time, in-person return to school this fall.

Clinics will be open to students at neighboring SPS schools. Dates and times for all clinics are listed below.

Students and family members can get either first or second shots at the June clinics!

These clinics are just one opportunity for our community to receive the vaccination, if your school’s date does not work, please contact your health care provider or see the list below for other vaccination opportunities throughout King County.

School Site Vaccine Clinics for SPS Students

The Pfizer vaccine is approved for people 12 and older and will be administered onsite at K-8, middle and high schools by members of the Seattle Fire Department.

    PARENT and CAREGIVER DIRECTIONS

K-8, middle and high school families will receive information from their school about their vaccination clinic date, time, and location.

Health and safety protocols, including mask wearing and social distancing, are required.

These clinics are for all students (both in-person and remote) who are 12 years and up.

WHAT TO BRING or COMPLETE BEFORE THE CLINIC

    Consent Form

Students will need to bring a signed parental consent form to the clinic. Consent forms will be emailed from each school. Printed copies can be requested from your student's school.

You may also download the form below.

Health Screening Day of the Clinic

All students will need to complete the SPS health screening before entering the building or clinic site.

If students participate in a clinic at a school other than their home school, that school should be noted in the health screening for the day.

Students may use the generic health screening before the student enters the school building. Generic Daily Health Screening Form

    REMOTE LEARNERS

Students who are participating in remote learning may arrive any time during the scheduled clinic block at their school. These clinics are walk-in only and there are no scheduled appointment times.

School staff will dismiss eligible students who have an approved consent form on file for the vaccination. Schools will communicate details of the in-person vaccination schedule to families.

The Pfizer vaccination requires two shots. Students should plan to be available for both school clinic dates to receive both doses of the vaccine.
