    Student-Athlete Forms

    Use the following guide to register your student for athletic programming. Once complete, these forms should be forwarded to the school where your student is participating.

    Persons with disabilities who require the documents on these pages in an accessible format, please e-mail athletics@seattleschools.org. Your request should identify the specific document needed and the format being requested.

    Step 1: Student Athletic Participation & Registration Forms

    Print, complete and return forms to school where your student is participating.

    (Note: Registration forms valid for one school year only; physical forms valid for 24 consecutive months.)

    Step 2: Sport Specific Guardian Release & Warning Forms

    For each school year, one form required for each sport student will participate.

    Step 3: Guardian Awareness Forms

    Print, complete and return forms to school where your student is participating. Information sheets should be read and referred throughout year, but Information sheets do not need to be turned into school.

  • Extracurricular Athletic Transportation Form
  • Weight and Fitness Room Form
  • Concussion and Sudden Cardiac Arrest Awareness Form
  • (If Applicable) Step 4: Enrollment Forms

    This checklist and forms it references are required for students not enrolled in an SPS high school. If you have any questions about the form, or if your student needs to complete this portion, please contact the school where your student is participating or Enrollment Office.

  • School Specific Forms

    • Schools may have supplemental forms they require their student-athletes to complete (in addition to the district required forms). Please contact the school where your student is participating to see if any extra materials are needed.
