For APPEALS only, testing provided by private evaluators will be accepted. These reports must follow the format listed. (We do not accept private testing in lieu of our official, Seattle Public Schools testing for initial eligibility.) Please note that, as per Superintendent Procedure 2190 SP, any data provided as part of an appeal will be considered as "an added data point in conjunction with the scores reported on the eligibility notification."

There are many evaluators in the Seattle are who are licensed to provide the types of testing you might want for an appeal. A google search should bring up a long list. For example Private Providers Search Results.

    Private Evaluators must provide:
    • A cover sheet (see below)
    • The full report
    • A copy of their license (with a current expiration date)
    The cover sheet must list:
    • Child’s full name
    • Birthdate
    • Test Data (in list or grid form, not a narrative)
The full report should begin on the second page and must be included.

*Approved Cognitive (intelligence) tests include current versions of the Wechsler Intelligence Scale for Children (WISC), the Wechsler Preschool and Primary Scale of Intelligence (WPPSI), Woodcock-Johnson Tests of Cognitive Ability, or the Stanford-Binet Intelligence Scales. The Appeals Committee will only consider results from intelligence tests administered within the last three years. We will also consider Verbal Reasoning and Quantitative Reasoning scores from the ERB if administered in a school setting no earlier than March 1 of the previous year.

**Approved standardized Achievement tests include current versions of Woodcock-Johnson Tests of Achievement, the Wechsler Individual Achievement Test (WIAT), and the Kaufman Test of Educational Achievement (KTEA). ERBs, MAP, and other standardized nationally normed reading and math achievement tests done in a school setting may also be submitted. We will only consider achievement tests administered on or after January 1 of the previous year.
