    What is the Affidavit of Marriage/Domestic Partnership Form? Do I need to fill it out? What do I do with it? The Affidavit of Marriage/Domestic Partnership form should be filled out if you will be adding your spouse or domestic partner to your healthcare benefits. Click on the task to download the form. You will fill it out by hand and have your spouse/domestic partner sign. The form should be brought with you to HR when you come in to complete your I-9 verification or it can be mailed to Seattle Public Schools, Human Resources - Benefits, Mailstop 33-157, P.O. Box 34165 Seattle, WA 98124-1166. Once you turn in, mark the task as complete on your onboarding checklist.

    If the form is not applicable to you, select “Skipped” from the drop-down menu on the task, which will cancel all reminders/emails and show it as completed on your check list.

    What is the OSPI Sexual Misconduct form and what do I do with it? All new employees must complete this form even if you never worked in a previous district. If you did not work at a previous school district, check that on the form and sign it. If you have worked for a previous district, complete the form and sign it.

    If you worked at more than one school district, click on the task “Download additional sexual misconduct forms” and print off the number you need.     How do I complete the W-4 form? How many exemptions should I claim? If you are unsure of the amount of exemptions to claim, you will need to contact a tax professional. SPS cannot provide guidance on your withholdings as it can vary greatly for every individual.

    I submitted a form and need to make changes. How do I do that?  If the form has not been locked (meaning the data has been acted on and entered into our systems), you should have the ability to update the contents by clicking on the grayed-out form link in your checklist.  Once a form has been locked, it means that the data has been entered into our systems and can’t be changed. If this is the case, once you have system login access, you can login to employee self-service to make updates to some information. Otherwise, you will need to reach out to the department responsible for the information (e.g., payroll, HR, retirement, benefits).

We highly recommend that you attend one of our Benefits Information Sessions, which you can schedule that by clicking here and selecting “New Hire Benefits Session” from the drop down.

    I see a task to complete Online Orientation and the Adult Sexual Misconduct Course, but I don’t see how to do it. The links to the required trainings are found under “Links to Required Trainings” section of the onboarding portal, to the left of the check list. Click the links and complete the course. Be sure to fill in your name at the end of the course to ensure you receive credit for completion.

    I am a current sub and have already filled out a lot of the new hire paperwork. Why am I being asked to do it again? When you become a staff member with an FTE of more than .5, you become eligible for benefits. In order to complete enrollment online, we send you to our onboarding portal, which also includes tasks that all new hires must complete. At this time, we are not able to distinguish the tasks based on subs who become full employees. We ask that you complete all required forms and skip those that allow it. You do not need to retake the orientation or adult sexual misconduct class if you already completed it when you were onboarded as a sub.  You may mark those items as completed in your check list. If you have not taken those courses, they are required.
