    Electronic Student Health Records

    CareDox is the district's secure electronic student health record system used by the school nurses and other staff to record health information.

     

    Updating your child's health information

    Please contact your child’s school nurse or the Health Services department at any time if your child has a new life-threatening health condition or allergies, or changes to their current health conditions/non-life threatening allergies, medications or treatments.

    Paper forms are also available. Some can be completed online, saved and then attached to an email. Other forms must be printed then either scanned and emailed, faxed or mailed.

     

    Online Access

    The CareDox portal for parent/guardians has been discontinued by the Health Services department.

    For school-year access to student health information, families continue to have the option to contact the school nurse or to contact the Health Services department via phone at 206) 252-0763, 206) 252-0750, or email at healthservices@seattleschools.org for health records.
