Welcome Westside Wildcats ’07-08! I am a WSHS staff member
and I share space with the Teen Health Center in Room 127 . I am
in my office Monday-Friday, 7:15am to 2:45pm.
Health
Registration Form
Please complete the enclosed Health Registration Form and return
to me. You can also find Health Services forms at www.seattleschools.org/area/healthservices/info/info.xml
I look forward to working with you and your student to manage
any acute or ongoing health concerns.
Medications
at School?
Students who take medicines either regularly at school or who need
any medicines at school on an occasional basis must have an 'Authorization
for Medication' on file with the school nurse. This includes over-the
counter meds like Tylenol ,ibuprofen, and allergy meds as well as
prescribed meds/ inhalers! A copy of the form is included in your
packet. Complete the parent sections on both sides, and have your
provider fill out the orders and return to me. If you include the
fax number, I will fax it to your doctor/clinic for their authorization
so you do not have to make a extra trip to your provider. This form
must be renewed each school year and allows students to carry their
meds or leave a supply, in their original containers, in the nurse
office. Students with inhalers and epi-pens must have demonstrated
correct usage to the nurse in order to self-administer.
If your student
was caught in a disaster and stranded at school for several days
would they need their meds? Do they ever forget their meds at home?
Families are strongly encouraged to keep a 3-day disaster supply
of meds (in original containers) at school with the nurse. Having
a back up supply is particularly important for students with inhalers,
epi-pens, diabetes and seizures.
Immunizations
Immunizations are important to keep all our students safe and healthy.
Moreover they are required for continued attendance at school. Immunizations
are available from your provider, at a Public Health Clinic or the
West Seattle Teen Health Center (a wonderful, on site resource).
The Teen Health Center is offering the HPV vaccine this year . The
Human papillomavirus can cause cervical cancer. (Refer to enclosed
handout)
In addition
to completing their Diptheria/Tetanus/Pertussis, Polio and Measles/Mumps/Rubella
series, incoming 9th and 10th graders must also have completed their
Hepatitis B series. Hep B is recommended but not required for grades
11-12. Varicella (chicken pox) vaccine is recommended but not required
for grades 9-12. Contact me for questions about your student’s
immunization status.
9th
Grade Scoliosis Screening
Scoliosis screening is required annually by law for all Washington
9th graders. Scoliosis is a side-to-side curve in the spine. A trained
screener observes the student’s posture while standing and
bending forward. Boys and girls are screened separately. If scoliosis
is suspected your student is re-screened at a later date. If a curve
is still suspected, you will be notified so that you can get a further
evaluation from your own provider. To refuse scoliosis screening
for your 9th grader contact me at 252-8807, leave your student’s
name and I will send you an exemption form to complete and return.
Vision
and Hearing Screens
Vision and hearing screens are not routinely done in high school
but will be done when a concern is expressed or upon request.
West
Seattle High School Teen Health Center
WSHS Teen Health Center is a safe, confidential place where students
can receive medical and mental health care. Operated by Puget Sound
Neighborhood Health Center, the THC is located on the main floor,
Room 127. Students must register at the THC before receiving services.
A parent or guardian must sign a consent form for the student to
be seen, except in the areas of mental health and/or reproductive
health a minor can self consent as allowed by law.
Free
and Reduced Lunch
If your student is eligible for free/reduced lunch PLEASE complete
and return the form in their packet. All students need the option
of a healthy breakfast and lunch to fuel their brains and be successful
at school. Students also qualify for a variety of fee waivers if
they have qualified for free/reduced lunch. PSAT, SAT, ACT and college
application fees are just a few! .
Strengths
and Difficulties Questionnaire (SDQ)
SPS Health program is now using the SDQ questionnaire to help find
students who can benefit from support, referrals, and extra planning.
When students are struggling academically it is especially important
to find out if there are barriers to learning that need to be addressed.
We are delighted to find a tool that is well established, accessible
to parents and older students, translated into many languages, simple
to use and helpful to guide decisions about the best student support.
There are many
circumstances where this tool may be useful for your child: academic
concerns, failing the WASL, needing a Student Learning Plan, being
reviewed by the Student Intervention Team. I can create a report
for your information or you can review the tool at: http://www.sdqinfo.com/
If you do NOT want the SDQ given to your child please contact me
at 252-8807, leave your student’s name and I will send you
an exemption form to complete and return.
