This form can help document the minutes of safety meetings in your workplace. This particular form isn’t required but shows the kind of information you need for your records.  You can either copy this form or make your own.

- Cover specific topics in your meetings.

1.  Unfinished business from last meeting:

Water Jugs:  some have been filled, we found more.  A watchdog dad will fill them

Fruit flies:  the problem is improving daily.  The compost goes out immediately after lunch daily.  Fruit fly traps are also in place.

Emergency supplies:  Mary contacted some Boy Scouts about doing an inventory.  They will let her know about their availabiliey

 2.  Any hazards reported during this time period?

Parents continue to park in drop-off zone.  We will continue our efforts to explain the safety rules.  Seattle Dept of Transportation will install speed cushions soon.  Joanne is working with Sea. Police to step up enforcement of speed regulations.

 3.  Describe any accident investigations conducted since last meeting.  Did you identify and correct the cause of the unsafe situation(s)?

 5.  What other safety-related topics did you cover in this meeting?

Incident command chart needs to be filled out.  We want to have it done before winter break.  Cheryl has staff surveys that should help fill in some slots.

Cheryl will print out lists of student emergency contacts and “sign out” sheets to put in the emergency supplies.

With no students next week, the IAs will go thru the classroom emergency backpacks to be sure everything is current and complete.

Not exactly safety but cost saving – we will remind staff to close window shades completely at night, as well as turn off computers and presentation stations.
