Reminders from your school nurseWelcome to McClure! My goal is to promote a healthy population and a healthy environment. To that end, please help us adhere to the following policies:ImmunizationsThanks for helping us meet the letter of the law (RCW 28A.210.160) by sending in your child’s immunization form! If your child was enrolled in Seattle Public School District last year, and you have never received reminder letters, you’re all set. If not, you should receive a letter from Health Services explaining the policy. In a nutshell, before the first day of school, your child must have up-to-date immunizations. If they are not current, or have not been submitted, your child will be sent home on the first day of school. No exceptions, no extensions, no excuses! You can download the form here. Or you can send me the dates for all DTP, Polio, MMR, and HBV vaccines. These are options to get them to the school nurse:
Health Registration formsPlease return a Health Registration form as soon as possible. If your child has a health concern that requires medication at school, the physician and the parent must sign the Medication Administration form even if it is an “over the counter” medication (including Tylenol, Aspirin, Ibuprofen, Claritin, etc). These health forms may be found on the school district website in many languages other than English. I look forward to meeting you. I will be at the new student orientation if you need to talk about any concerns you have about your child’s health at school. Feel free to call me once school starts if necessary.
