Volunteer Driver Checklist
Parents/Guardians must fill out this Seattle Schools Volunteer Driver Checklist for the first time that they drive on a field trip in a school year.
Request for Driving Record
This is the State of Washington Department of Licensing Form that we must use for parents who drive on two or more field trips in a school year. The good news is that you ony need to fill out one each year. The form should be returned to Lowell, not directly to the Department of Licensing as indicated on the bottom of it. The School District Risk Management office will then send the forms to the state for processing.
