Laurelhurst PTA and the Laurelhurst Elementary School Library would like to invite you to dedicate a book in honor of your child’s birthday, graduation or any other special event.  All dedicated books will have a nameplate honoring the child and the occasion inside the front cover.  This is a wonderful way to celebrate your child and provide a lasting keepsake for others to enjoy for years to come! 

If you would like Mr. Brown to select a book in honor of your child, please attach a check for $17.00 made payable to the Laurelhurst PTA to this form and place it in the Birthday Books folder in the Office, or return it via Kid Mail to the school.  If there is a specific title or subject your child would like to have in the library, please check with Mr. Brown to make sure it would not be a duplicate and then purchase the book yourself and bring it to the library with this completed form.
