Fairmount Park Elementary School

Individualized Student Learning Plan

Value Statement:  In order to close the academic achievement gap, students at Fairmount Park are monitored every 8 weeks.  The classroom teacher and grade level teams set goals for all students.  When a child has been identified as needing more than one year accelerated growth, or is at a level higher than most students, an Individualized Student Learning Plan is used.  This plan is discussed with the teacher, principal, and parent and is mutually agreed upon.  This plan is meant to close the achievement gap only, and is not to be used as a behavior plan, though behavior goals may be attached.

Fairmount Park 2006-2007 Making Consensus:

1.    A consensus decision is a decision made for staff and committee meetings.

2.    Every opinion will be respected.

3.    Full staff participation in discussions

4.    Meeting minutes are to be taken and posted so as to keep all informed.  This will be true for committee meetings as well.

The Model:

            5 fingers = strongly agree

          4 fingers = agree

          3 fingers = neutral

          2 fingers = disagree and won’t interfere

          1 finger   = strongly disagree

Fist  =  complete disagreement to the degree that one would sabotage the efforts.
