Athletic Participation Requirements

Athletic Sport Packets

 

For those students needing to complete a physician's physical, that form can be accessed (and printed) here.

The following items are included in a complete sports packet:

  --A completed sports physical
    1. Sport physicals are valid for 24 months
2. Incoming 9th graders physicals are valid, if not expired
3. Chief Sealth Teen Health Center Nurse is authorized to administer sport physicals.
     

Any student enrolled at Chief Sealth, West Seattle, Roosevelt High School, and Madison or Denny Middle School can be seen at the respective health center by having a parent or guardian complete and sigh a registration form. Registration forms can be obtained at the student health center

Puget Sound Neighborhood Health Centers believes that quality health care should be available to everyone, regardless of their ability to pay. They accept Medical coupons and private insurance and can also help eligible families sign up for insurance plans.

    1. Parent and Student read to understand the expectations and requirements for participating in a sports program in Seattle Public Schools
2. The manual is not inclusive of all Seattle Public School District and WIAA directives but is intended to serve as a resource to help parents and students with common questions and procedures. For further questions, visit the WIAA at (www.wiaa.com) or SPS (www.seattleschools.org/) to gain more in-dept information pertaining to participation in athletics.

    1. All Chief Sealth students-athletes are expected to follow and promote Commitment to Conduct, to Team, to Academics, to Language, Self, Sportsmanship and to School

  --Insurance
    1. In order to participate in athletics, all students are required to have some form of injury/accident insurance to help defray costs resulting from any injuries which may occur as a result of participation in athletics.

Pay to Play

  To best support our high school sports programs and student athletes, and consistent with other school districts around the region, the Seattle School Board approved an Athletics Sports Participation Fee Policy effective beginning with the 2007-2008 school years.
 

Fee Structure:
- 1st Sport Participation: $50.00
-
2nd Sport Participation: $25.00

Free & Reduced Lunch Students
- 1st Sport Participation: $25.00
- 2nd Sport Participation: $12.50

Three sport participants capped at: $75.00 or $37.50

    Requirements to be eligible for Fee Wavier:
      - If two or more siblings in the same household are participating in a sport during the same season, the Pay-to-Play fee is waved for that season.
- Parent(s) are living in emergency shelter.
  Collected Athletic Sports Participation fees do NOT guarantee a spot on the team or ensure equal playing time.
  - Fees will be collected simultaneously with completed sport packets. *Sports packets will not be accepted without an associated fee.

ASB

  Chief Sealth High School Athletic Department requires that ALL students participating in a sport must buy an ASB card. No refund is available.
    -Fee: $25. Free and Reduce Lunch Household $10.00

Refund Policy

  Refunds will be provided ONLY under the following conditions (refunds must be requested prior to the end of the sports season):
    1. Student is cut from the squad by the coach: 100%
2. Student quits due to illness or injury prior to 1st contest:100%
3. Student quits due to family moving prior to 1st contest: 100%

Eligibility

  Be under twenty (20) years of age on September 1 for fall sports season; December 1 for winter sports season; and March 1 for spring sports season. Effective Fall 2006, all students (9-12) participating in athletics must:
    - Maintain a 2.0 grade point average and earn 2.5 credits during the previous term to be eligible to play. Student-athletes who do not meet the above previous term credit and GPA requirements:
      1. Must have a five-week grade check conducted five weeks after the beginning of the term, not five weeks after the start of the season or five weeks after the student turns out for a sort.
2. Must have a five-week grade check every five weeks for the duration of the season or (term if participating in two sports)
3. In order to maintain eligibility each five-week report must indicate that the student is receiving 2.5 credits with a minimum of 2.0 GPA
    - Reside with their parents, the parent with legal custody, or a court appointed guardian who has acted in such a capacity for a period of one year or more.
- Not miss practices or games for the purpose of participating in non-school athletic activities without league approval.
- Not accept cash awards in any amount or merchandise of more than $300.00 in value, or have ever signed a contract with or played for a professional athletic organization.
