Student Athletic Participation Registration Forms
Print completed form, sign and return registration form to school.
(registration form valid for one school year, physical form good for 24 consecutive months)
Athletic Participation and Physical Registration form

Sport Specific Parent/Guardian Release Form
(one form require for each sport student will participate in for each school year)

Private School & Homeschool Based Instruction
Cross Enrolled Athletic Students
- If student attends a private school or is homeschool based instruction and has met all cross enrollment requirements in their assigned area school the Seattle Public Schools Enrollment form needs to also be completed.
