Student Records
Transcript, Academic History, or Diploma Requests
Current students contact the school for the student's cumulative file folder.
To order a certified copy of a student transcript ($15.00) or a replacement diploma ($30.00) submit a Request for Student Records form along with
2) Signature of Student (if over 18) or Parent-Legal Guardian
Where the individual is deceased, please provide proof of death, either a copy of the death certificate or published obituary.
For birth date verification social security, passport or other purposes, please use the Request for Student Record signed by the former student.
For elementary school records, please email the name of the student, date of birth and your contact information to Student Records so we can determine if the record exists.

Verifications of High School Graduation for Employers
Send a FAX (206)252-0786 using your company letterhead to Student Records Office with:
- Your name, firm, telephone number and address.
- Email address where you can be reached for questions.
- Name and birth date of former student (name used in school).
- School(s) and graduation or attendance date(s) to be verified.
Faxed verifications cost $15.00, remitted to Seattle Public Schools, Student Records. It takes 3 - 5 business days for a response.
Special education records are provided through a different department.
Replacement Traffic Education Certificates are provided by the Student Records Office.
