Insurance Verification
If you have successfully completed both the classroom and
behind-the-wheel driving portions of a traffic education course in
a program provided by the Seattle Public Schools and need
verification for your insurance company, we can provide that
information.
Send a request authorizing the Seattle Public Schools to release
the information to your insurance company. The request must be
signed by you (the student) and by a parent or guardian if you are
under 18 years of age.
Include the following information:
* Your full legal name.
* Date of birth.
* The school where you completed the course.
* The FAX number of your insurance company and the name of the
person to whom it should be sent.
* A daytime phone number or email address of a custodial parent or
guardian (in case we have questions).

We will verify your completion of the course and FAX that
information to your insurance company. Please allow one week
for this service (three weeks if school is not in session).
