Birth Date Verification for Social Security, Passport, and Other Purposes

For birthdate or other verification from school records for social security, passport, or other purposes, please send a written request signed by the former student.

There is a $5.00 fee for each student record requested.

Include the following in your letter:

• Your name or names used while attending school
• Your birth date
• Your parents' or guardians' names for research verification purposes
• The names of the schools and approximate dates you attended
• A copy of your driver’s license or your picture identification that includes your birthdate
• If applicable, your petition for change of name and court order changing name
• Your handwritten signature
• A check or money order payable to Seattle Public Schools for $5.00 times the number of record copies ordered.
• A number to contact you in the event there is a problem locating the record
