Science Materials Center.

submitting your order, print the summary for your records, if

materials are provided only to teachers who have the science unit
for which those materials are intended.
- Please coordinate
delivery dates with other teachers needing these materials at your
school. Deliveries may be scheduled for the dates above
ONLY.

completely fill in the delivery information below. Live materials
cannot be delivered without this information.

Our vendor guarantees
the live materials being delivered to your school.
You must call the SMC at (206) 545-7024 within 24 hrs if
there is any problem with your plants or animals so we may replace
them for you at no charge.

PLEASE
NOTE:
Due to Washington State Environmental Regulations, live
materials
provided for use in your science kit may not be released into
the
environment. Please contact the Science Materials Center if you
need any
information regarding disposal of live materials or plants. Thank
you.
