NOTICE

LIVE MATERIALS ARE ON THEIR WAY TO YOU!
For the 2008–2009 school year, all teachers receiving Animals 2x2, Organisms and Ecosystems science kits by the first day of school will have their live materials deliveries pre-scheduled for your convenience. You are not required to submit this request form in order to receive your live materials unless you have recently taken training for these titles or did not receive your science kit at the start of the school year.

The pre-scheduled dates of delivery of live materials to your school are as follows:

Animals 2x2 # 1 & # 2 - September 24-25
Animals 2x2 # 3 & # 4 - October 22-23

Organisms # 1 – September 24-25
Organisms # 2 – October 8-9

Ecosystems # 1 – September 10-11
Ecosystems # 2 – September 17-18

If you have recently been trained on these titles or were issued a science kit after September 2nd and need to order live materials at this time, please continue to the live materials order form. You will receive an email confirmation once your order has been received by the live materials coordinator at the Science Materials Center. Instructions for submitting your order for SEA-ANIMALS#1:

Complete the following form. Be sure to include the names of each teacher receiving a kit.
Review your request.
Click the Submit button to e-mail your request to the Science Materials Center.
After submitting your order, print the summary for your records, if needed.

Live materials are provided only to teachers who have the science unit for which those materials are intended.
Please coordinate delivery dates with other teachers needing these materials at your school. Deliveries may be scheduled for arrival on Wednesday or Thursday ONLY.

Our vendor guarantees the live materials being delivered to your school.
You must call the SMC at (206) 545-7024 within 24 hrs if there is any problem with your plants or animals so we may replace them for you at no charge.

PLEASE NOTE:
Due to Washington State Environmental Regulations, live materials
provided for use in your science kit may not be released into the
environment. Please contact the Science Materials Center if you need any
information regarding disposal of live materials or plants. Thank you.
