In order to be listed under the Seattle Public Schools Small Works Roster, please complete the Small Works Roster Application Form and submit it to the address or fax number listed on the application

Attachments for Small Works Roster Application Form (to be submitted within 7 calendar days of notice of intent to award )

  2. Intent to Pay Prevailing Wages (Please note that updated forms are available from L&I and the Filing Fee is now $40)

  6. Quote Form:  Lump Sum Project
