Q: Seattle School District sends out a letter addressed to high school students, asking whether they want their contact information to be sent to the military. Does this need to be filled out every year?
A: No, this needs to be filled out only once in high school.
Q: Does the Family Educational Rights and Privacy Act (FERPA) form need to be filled out every year?
A: Yes. This form needs to be filled out every year the student is in school, if the family does NOT want the student’s name and contact information shared with parent groups, colleges, prospective employers or the military. The due date is October 1.
