For your convenience the 4 forms that must be filled out by the provider and/or require input and/or signatures of school or district administrators are available, for the first time, in a format designed to download and complete on your computer. Once completed the forms, Planning Packet Cover Page, the K-8 and Middle and High School Alignment Partnership Rubrics and the School / Afterschool Shared Resources Plan must still be submitted in hardcopy (4 sets of copies) along with all other required documents and forms by the deadline, April 30, 2009. Learning Partners Group Plan Reviewer's Comment form is posted here for your reference only. This form is completed during the formal plan review process by designated members of the Learning Partners Group, and qualified educators, as needed. Blank forms are provided at the time of reveiw. Letters to Providers and Principals regarding the status of each Community Alignment Initiative Program will notify the sites whether or not each is approved for the upcoming school-year.
