Community Alignment Partnership
Packet Materials
2009 - 2010 School Year
For your convenience the 4 forms that must be
filled out by the provider and/or require input and/or signatures
of school or district administrators are available, for the first
time, in a format designed to download and complete on your
computer. Once completed the forms, Planning Packet Cover Page, the
K-8 and Middle and High School Alignment
Partnership Rubrics and the School / Afterschool Shared Resources
Plan must still be submitted in hardcopy (4 sets of
copies) along with all other required documents and forms by the
deadline, April 30, 2009. (See Packet

Page for detailed instruction on submitting
packets.)
Our thanks to Learning Partners Group member Kyla
Lackie, Youth Program Manager for SOAR. Kyla joined the Learning
Partners Group this year with much to offer, including the ability
to create the Word format forms below.

Partnership Packet includes Alignment Report as one of the required
documents Providers submit. |

Alignment Report: At this time
the Excel Spreadsheet is available via email. Alignment Providers
may request the form by sending an email to Susan Hall @ slhall@seattleschools.org
with the Subject: Alignment Report
Request.
Adobe PDF documents are provided here for reference and as
information accessible by any interested party regarding the data
aligned providers submit to the Seattle Public Schools Office for
Community Learning.

Additional documents
supporting plans may be submitted by the provider as part of the
complete packet.
One excellent example submitted by
Blazing Trails for the 2008 - 2009 school year outlined Practices Supporting the 40
Developmental Assets.
Documents not included in plan or
packets, used only in the office during process of
review.
Learning Partners Group Plan Reviewer's Comment form is posted
here for your reference only. This form is completed during the
formal plan review process by designated members of the Learning
Partners Group, and qualified educators, as needed. Blank forms are
provided at the time of reveiw. Letters to Providers and Principals
regarding the status of each Community Alignment Initiative Program
will notify the sites whether or not each is approved for the
upcoming school-year.
