To enroll your child, you will need the following documents:
- Application and registration forms. Available
at the Bilingual Family Center or download from our website under
Enrollment/Forms.
- Identification. Picture identification of the
parent or guardian is required, such as driver's license,
Washington State Identification, or passport.
- Proof of address. Two pieces of identification
are required for address verification, such as signed and dated
telephone, utility or cable bills; lease or mortgage information;
insurance documents; Court documents; or documents from public
agencies such as DSHS. If living temporarily with someone, you need
to complete, sign, and notarize the Temporary
Residence Form. (You can also obtain this form from the
Bilingual Family Center).
- Child's immunization record. Dates provided
must meet current state health requirements.
- Proof of birth date. State or hospital birth
certificate, current DSHS card, I-94, or passport.
When you have gathered the necessary documents, bring your child
to the Bilingual Family Center for language assessment.
A child who is 5 years old by August 31, 2008, can attend
kindergarten and should apply during the on-time
enrollment period (January 22 to February 29, 2008, for the
2008-2009 school year).
