Two versions of the survey are available: PDF and Word

The PDF version can be opened and printed. Please compete and return to your school no later than Friday, February 11th.

The Word version can be opened, and saved to your computer, so you can complete it and then submit electronically to Dana via her e-mail: dcnacskms@hotmail.com. Dana must receive the survey no later than Tuesday, February 15th.

Late surveys cannot be included in the analyses that will inform the Spectrum Task Force's recommendation to the Program Placement Committee; however, we will add late surveys to our database.
